Your Learning Portal is proud to offer Health Assessments as part of our services.
This service is led by our in-house Registered Nurse who is able to assist you with your assessment.

1. Client-centred Health Assessments, including, Bladder and Bowel Continence Assessments
Individually tailored health assessments conducted by our highly qualified Registered Nurse, who will be able to identify areas of concerns and contributing factors.
2. Individualised Health Support Plans
Each management plan is developed against the individual needs of our clients, including lifestyle goals, to ensure practical and achievable outcomes.
3. Ongoing Review
We provide short-term follow up and reviews based on your individual needs, to ensure the best outcomes for you.

Norah, our healthcare professional has been working in the healthcare industry both in Australia and overseas for over 45 years.
She has a range of qualifications that eminently position her to be able to undertake your healthcare assessment, provide a robust person-centred report and healthcare plan to meet your individual needs.
Norah is a Registered Nurse with a range of qualifications that enable her to provide clinical care services to people living in community settings and can visit you in the privacy of your own home to conduct your assessment for a range of health needs.
Norah is able to accept referrals for any person across the lifespan including children, men and women of any age.
Referrals may be accepted from any person and you do not need a doctors referral to access our services.

For many chronic health conditions an ongoing nursing assessment can be a good way to ensure that things are on track or that you have the best management plan in place.
You may also not have had an assessment in a long time and just want to know if there is any other management options.

This may depend on the type of funding that you receive.
However, if you do not receive any funding you may need the assessment to apply for funding.

You do not need a doctor’s referral to access our health assessment services.

You may make an appointment by calling us on (08) 7113 3071, 0411 849 194 or email us at admin@yourlearningportal.com.au.
The appointment is usually completed face to face at your home, however you may choose to have an assessment over the phone.

After the assessment the nurse will need to write your report.
The report may take up to 3 weeks to complete.
A copy of the referral will be sent to you and any other person you may nominate such as your GP.
A follow up appointment will be booked after the assessment.
